ช่วยแปลให้หน่อยครับเข้าให้ส่งเอกสารอะไรไป

คือผมเป็นสมาชิกของเว็บไชต์แห่งหนึ่งเขาอยากได้เอกสารแต่ผมแปลเองไม่ค่อยชัวร์ว่าเขาอยากได้อะไรบ้างช่วยแปลที่ครับ
